ELECTRONIC OIL RECORD BOOK (PART I)
 

Requirements to obtain of the Annual Licence for the use of the Electronic Oil Record Book (Part I) aboard a Panamanian registered Vessel

• Application form duly filled and signed [download form]
   
• Copy of the vessel’s Navigation Licence (Patente)
   
• Copy of the vessel’s current Annual Tax Receipt or Tax Clearance Certificate
   
• Letter Declaration from the Manufacturer indicating and confirming that the e-book was purchased to be used on board the vessel. [See specimen]
   
• Letter Declaration from the shipowner or ship manager confirming:
  • That installation on board has been done according to the Manufacturer instructions.
     
  • That officers onboard have received training according to STCW-95 rule I/14.1.4,
     
  o Those routines for safekeeping of the electronic logbook are according to regulations from flag state and IMO and are described in the vessel ISM system. [See specimen]
   
• Remittance to cover the Licence’s official fees, administration charges and courier expenses. [see bank details]
   
• Check List detailing the documents enclosed and the instructions where to dispatch the Licence once it has been obtained. [See specimen]
